The Upper Canada Family Health Team (“UCFHT”) will review all feedback provided within ten business days. Where the person providing feedback has also provided contact information, the UCFHT will provide a response by an appropriate method (usually in writing, unless some other form of communication is requested) within an additional ten business days.
