Upper Canada Family Health Team

​

The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. Measurable goals include reducing the number of patients in Brockville and the Thousand Islands who do not have a health care provider, reducing ER visits for minor illness, and reducing hospital admissions for chronic illnesses.

The Upper Canada Family Health Team is comprised of 2
6 physicians who work in groups of two or three over 10 different sites serving approximately 36,000 patients. Working collaboratively within the team are allied health professionals and nurse practitioners providing primary care and programs, which include:

  • Nutrition

  • Foot Care 

  • Memory Clinic

  • Mental Health

  • Preventative Care

  • Lung Health including Smoking Cessation

  • Coordinated Care Plans

The Upper Canada Family Health Team is funded by the Ministry of Health and Long Term Care.
