Program Details

The Pharmacists will be doing medication reviews for patients prior to each Memory Clinic and Dr. Power (Geriatrician) clinic days. One of the pharmacists sits on the Memory Clinic team and reviews all of the medications with the patient and caregiver during their appointment. Providers can refer patients to the pharmacists for medication reviews and can also provide assistance with drug information questions.
